Breakthrough House Residential Services seeks to provide a living environment conducive to psychosocial rehabilitation for persons experiencing mental illness as they move toward recovery and independence
The Breakthrough House Residential Services Program provides safe, affordable, housing for individuals with mental illness. Program participants have the choice of living in a supported living group home, a semi-independent apartment, or an independent apartment, depending upon their individual needs and preferences. All locations are community-based.
Supported Living Group Homes
Our supported group living environments help decrease isolation, reduce hospitalizations, and improve socialization. Residential Services staff members work with each resident and their case manager to establish goals for the resident’s activities of daily living, develop an individualized plan for attaining these goals, and assist the resident in carrying out their plan. The approach is collaborative, person-directed, and individualized. The focus is on helping individuals re-develop skills and access resources needed to increase their capacity to be successful and satisfied in the living, working, learning, and social environments of their choice. In addition to staff supervision, some of the many services provided include meal planning, grocery shopping, medication monitoring, group activities, and limited transportation.
Independent and Shared Apartments
Our independent and shared living environments are for individuals seeking minimum support while still maintaining contact with staff.
Residents are responsible for their own activities of daily living, including housekeeping, without staff supervision. The Breakthrough House, Inc. wellness coordinator visits each resident in their home on a regular basis to monitor and ensure the safety and general well-being of the individual as well the safety and sanitation of their home. The wellness coordinator also serves as a liaison between community health providers and residents and can assist them in making contacts and referrals when indicated.
Who Is Eligible?
To be admitted to the Breakthrough House, Inc. Residential Services Program, an applicant must meet the following requirements:
- Be 18 years of age or older
- Have an Axis I (primary) diagnosis of mental illness
- Have the ability to understand and comply with program guidelines
- Have a current mental health provider
- Have current and active case management
- Have a 30 day history of taking psychotropic medication as prescribed by mental health provider
- Not be currently or actively using illegal substances
- Successfully pass Residential Service’s screening and background check
- Not be a registered offender in any state
All information provided, including income and medication compliance, will be verified prior to
acceptance into the program.

Qualified persons must complete our Admission Application and return the original along with the necessary documentation (listed on the Admission Application) to the Breakthrough House, Inc. Administrative Offices, Attn: Admissions Committee, at 603 SW Topeka Boulevard, Suite 100, Topeka, Kansas 66603.
